Too many changes too often that's why there are no core practices. As soon as the one framework is rolled out and is in operation 6 months later the new change takes over and this goes on and on every 6 months. Good planning and delivery happens only in pockets. Coordination/collaboration between some departments is so scarce that miscommunication/misalignment is inevitable.

The company lacks focus and ethics. Sales consistently over promises and generally has no clue what the software can and cannot do (or they minimally act ignorant). The company gets by entirely on custom engineering work per customer which is hastily completed and unmaintainable at scale. The product is riddled with issues to the point that affects study data integrity. And meanwhile management (100+ VPs and C-suite) is out chasing the next buzzworthy vaporware opportunity.
