Pervasive culture of blame, everyone is out to cover their butt and blame someone else if something goes wrong. Which is odd because generally speaking things go fairly well. The incentive structure is set up the measure some departments productivity and to increase that metric it often sets one department against another. In other words we spend more time "in-fighting" then servicing our customers.

Lack of real leadership talent at most levels of the organization. Technically proficient managers are abundant but managers who can lead people are scarce.
