Over communication regarding benefits (always high level) making it hard for the employee to understand. Poor communication when making internal changes to procedures. Inconsistent compensation among non operation staff.

There was no room for advancement. The people involved in giving you a raise or a promotion had no idea who you even were. The project managers who actually dealt with you day to day had very little power to control this. After 4 years doing the same job I made a whopping $2 more per hour near the end of my tenure, and percentage wise I was getting the highest raise on the team!
