Benefits are VERY expensive, You cant afford it :( but you have to have it. I pay almost 700.00 a month in premiums for a family of 3 and the copay and deductibles keep going up!

I have Banner Choice Plus, which is the most premium plan you could choose, and the $ 50 copay to see a specialist seems over the top for me for how much I pay a month

Very good coverage with minimal coinsurance if using the Banner network. However higher copay and premiums than local competitors. Premiums are very high and there's a large smoker surcharge with no allocations for participation in a cessation program. Many people lie about smoker status, and if you do honestly mark your status as a smoker, it is a lengthy time to be removed (over six months) once you are tobacco free, which can equate to an extra 2400 if you have a family plan.

Very reasonable co-pays. ER co-pays were waived if hospital admission. It was self funded. Required primary care referral to see specialist.

Nothing is terribly wrong with their health insurance, problem is even being seen at a banner health facility you still have high deductibles.

Just as expensive as any outside health plan to get care at their own facilities. Poor coordination between plan administration and billing departments despite all working for the same company. Plan benefits continue to become more expensive every year with reduced benefits.

i don't have it because i am per diem but i have heard it is not that good, you have to go to only banner brand hospitals and doctors and sometimes they are slow.
