Eligible employees may make pre-tax contributions toward their group health plan benefits, have a choice of in- and out-of-network coverage and pay a flat office visit co-payment. Depending on the health care insurance carrier, participants may or may not need to designate a primary care physician. Prescriptions will be filled at any participating pharmacy in the U.S. for only a co-payment fee. There are no claims to file at participating pharmacies.

pretty decent but they have prescription deductibles that I've never had before and it's a huge inconvenience. Also, you cannot add people to just vision or dental. It must be grouped.
