It was the type of consumer choice insurance meant to cover emergency or urgent care and set you up with a HSA account which the employer contributes to during each pay period. It get a discounted rate at enrollment time each year, you had to go have a health screening and fill out a questionnaire which was all paid for by the employer.

Many providers won't accept their provided insurance. Extremely high out-of-pocket expenses, forced to use bad mail-order pharmacy. It's cheaper overall to get your own insurance through the Government Exchange. Charges $1,000 extra to add a spouse/domestic partner. Forced health check-up and quit smoking program or pay $600 dollars extra.
