At Pediatric Home Service (PHS), we are united by a shared mission: to help medically complex children live their best lives, at home and in their communities. As a national leader in pediatric home care, we deliver a full continuum of services that empower families and elevate outcomes. Pediatric Health Choice (PHC), a proud member of the PHS team, operates 16 Pediatric Prescribed Extended Care (PPEC) centers across the country. These centers provide a safe, nurturing environment where children with complex medical needs receive expert nursing care, therapy, and socialization in a day health setting. PHC is more than a care provider, it’s a community. Our centers serve as a haven for families, offering respite, support, and peace of mind. Together, PHS and PHC form a nationally recognized Pediatric Center of Excellence (PCoE). This reflects our multidisciplinary approach, our investment in care innovation, and our unwavering focus on outcomes and patient satisfaction. Headquartered in Cook, Minnesota, Scenic Rivers Health Services provides patient-centered medical, dental and behavioral health care to remote, underserved areas in northeastern Minnesota. Our dedicated providers and support staff ensure the well being of their communities across the northland. We recognize that our patients are the reason we are here, and we know it’s our job to keep our rural communities healthy. Thank you for your confidence in our providers and staff. As a non-profit organization, our providers’ only goal is to help you get well and stay healthy. Our status as a Community Health Center ensures that our staff will do everything to help eliminate the financial barriers between your family and the care they need, including a Sliding Fee Scale program. Scenic Rivers collaborates with Cook Hospital and Bigfork Valley Hospital to provide Critical Access Hospital services at its Cook and Bigfork Medical Clinics.
