Located in Bolivar, Missouri, CMH is a rural healthcare system serving eight counties in southwest Missouri. A three-time Missouri Quality Award recipient, CMH is a Level III Trauma Center, Level II STEMI Center and Level III Stroke Center. CMH also has 34 primary and specialty physician clinics, seven long-term care facilities and other ancillary services. CMH has started a 117,000-square-feet expansion and 25,000-square-feet remodel project! New parking lots have been built, our mobile kitchen has arrived and demolition has started. Haven Healthcare is a multi-faceted health care provider with expertise in variable degrees of care. Haven Home Health LLC is a Joint Commission accredited home health agency with a focus on rehabilitation programs. Haven Hospice LLC is a Joint Commission accredited hospice with a strong palliative care approach. Haven Rehab is managing and staffing rehab services to skilled nursing facilities in Missouri. Customer service, CMS regulations, billing oversite, caseload development and documentation QA are just a few of the services we provide to our skilled nursing customers. Haven Physical Therapy LLC provides outpatient services with a manual therapy focus.
