We are committed to helping individuals with intellectual disabilities, mental health challenges, and seniors realize their full potential by providing personalized, compassionate support tailored to their unique goals and aspirations. Our commitment to quality care has earned us recognition from the Commission on Accreditation of Rehabilitation Facilities (CARF) since 2003. Our Services and Impact

Meridian Services offers a comprehensive range of services to people in need. Our programs and services are designed to foster independence and promote personal growth. We are proud to serve a wide geographic area, including the Twin Cities metro area and smaller counties across Minnesota.

Currently, Meridian Services supports more than 5,700 individuals and their families. Our Case Management Department assists approximately:

• 2,500 elderly individuals
• 1,500 people with mental health diagnoses
• Over 1,000 individuals with intellectual disabilities

In addition to our case management services, we provide care in 30 residential group homes across Minnesota. We also offer In-Home/SILS/ILS services and respite care to over 150 individuals, ensuring that our clients have the support they need to thrive in their communities.
