Underqualified people are hired, but are not given adequate training. The department I worked in would change the accounts of the failing workers and give them to the qualified workers. So the relationships never develop between the home office and the hospital, clinic or lab. Once the accounts were cleaned up they went right back to lesser skilled workers.

Low morale, high turnover, high stress, difficult work/life balance due to mandatory overtime in some departments, and "good ol' boy"system for many assignments and/or promotions. Extreme focus on meet daily goals by "touching accounts" (i.e., noting or status) vs. a true focus on account resolution (getting patient accounts paid).
