      People are usually open to collaboration and supporting one another.

      Too many changes that aren't well thought. Employees spend a great deal of time and effort fixing or finding workarounds for things that used to work fine until someone (usually far removed from the actual end process) decided to change it. Most things are just a checkmark, and don't work as designed.
