Extremely weak leadership at the top. The whole place is run by only a few people who are very outdated in their thinking. Extremely weak HR and Talent Management practices. Some very poor decision-making at the top.

Honestly, not much I can think of at this point. There were some teams (and individuals) that did not work well together in the past, but those individuals have largely been moved out and the organization has helped with tools to promote teamwork and communications.
