Executive Management's leadership skills are at best, once present and currently atrophied, at worse (and most likely) never existed in the first place. Leadership (from SVP up to CEO) lacks self awareness, accountability, instead takes pride (it seems) in pointing fingers and naming scapegoats. VPs and below are required to take it in the ... expected to be blamed and "leave if we don't like it". (and we are)
