Very flat organization, very little room for growth. Supposedly a "matrix" organization, but no one there knows the actual organizational structure, and they prohibit org charts. This makes cross-functional communication nearly impossible unless you have been there for years. Very high turnover rate. Constantly shifting "strategies", like a new one every six months. Somewhat unreasonable expectations from upper management. Nepotism runs rampant with generally negative impacts.

Middle management who haven't a clue what they're doing. Individuals being promoted from within, without the necessary acumen to fulfill the roles they have. An HR department, that stalk around the work place and act like they rule the place. Senior management so out of touch and so far removed from the day to day running of the business, that questionable decisions are being made on a constant basis.
