Lack of real management. Red Tape. Too many cooks. too may contradicting directions because of certain peoples personal interests. teams forced to compete against each other. no real grasp of target market from marketing leaders. no innovation...forced to do same tactics and more repetition hoping to get a better result (def of insanity) an HR that fails at keeping a safe and proper conduct business environment.

Upper management doesn't have clear direction. They expect employees to work extended hours when you are salaried, as 40 hr work weeks just don't cut it. Doesn't matter if the work is getting done, it's all about appearances and whether you're seen as the first one in and last one out.
