- Too much focus or process and not progress: a minimally invasive production change requires a significant amount of paperwork and approval (ex: JVM logging change to set logging levels through JMX could require 20 minutes of paperwork, approvals from multiple people, reviews, and more: hours of work generated from a 2 second change) - Claim to be agile, but ITIL processes get in the way - More work, less people - Unclear, unrealistic expectations - Large, nebulous, projects with shifting priorities - Open office concepts - Too many transient bank managers running an IT company like a bank
