Management, high turnover rate, on call requirements, work/life balance (you will basically work 2 week's straight when you are assigned on call for a weekend, it's just added on top of your existing weekly patient load), poor communication between departments, long hours (often you will work a full day then be on call all evening/night, many times did 14-16 hours -any after hour appts, like late discharges from hospital are put to on call so you will be out doing visits until like 9 or 10pm then have to answer calls all night and potentially make visits in middle of night), high amount of travel, less than average mileage reimbursement and nothing towards wear and tear on you vehicle, poor pay for amount of responsibilities, no assistance when sick (you spend all day getting your patients reassigned), promised many things during interview that were not followed through. Beware!
