Crisis of decision making at every level. Leadership is not in touch with realities of an overworked and demoralized workforce, especially physicians, who feel talked down and dictated to much of the time. Too much physician time is being spent on mundane clerical tasks and very little on actually "talking" with the patient.

Unrealistic corporate goals, lack of ownership of core programs, high rate of executive turnover, factions vying for leadership leads to lack of accountability. A conservative, highly structured organization that is afraid to speak truth to an executive group that doesn't want to hear it.
