**Understaffed and Overworked** Employees are set up for failure, everyone is operating at maximum capacity and yet workload continues to pile up. There are not enough resources to complete the work efficiently and effectively. Everybody is burned out. **No Systems, No Procedures, No Planning** The site is not organized and always in reactive mode. The focus is always about ‘getting product out the door,' regardless of sustainability. Projects and deviations are given unrealistic deadlines. There is little investment in real continuous improvement or process optimization. **Toxic Blame Culture** When deadlines are missed, the finger-pointing begins. People are quick to blame other departments. Those in the inner circle are able to deflect responsibility onto burned out employees and hold no accountability. **Leadership Disconnected or Absent** Leadership does not recognize the effort to make the impossible possible. Leaders spend more time talking about the work, rather than facilitating the work or granting resources.
